Community-Based Supports provide one-to-one assistance to help individuals with intellectual and developmental disabilities achieve greater independence, productivity, and community participation.

What the Program Offers

  • 1:1 support in the community or home
  • Educational, enrichment, and recreational activities
  • Opportunities to practice daily living skills and social engagement
  • Flexible services tailored to each participant’s goals

Who Can Participate

  • Individuals enrolled in the DDD Supports Program
  • Available in Camden, Essex, Gloucester, Middlesex, Monmouth, Morris, Ocean, and Union counties

How It Works

  • Referral source: DDD
  • Services billed through participant ISP
