Support Coordination Services in New Jersey

Navigating New Jersey’s Division of Developmental Disabilities system can feel overwhelming. You do not have to do it alone. 

At Caregivers of New Jersey, we provide Support Coordination services for individuals with intellectual and developmental disabilities who are enrolled in the NJ DDD program. Our experienced support coordinators help individuals and families understand available services, build Individualized Service Plans, and connect with trusted providers in their local communities. 
 
We proudly serve individuals and families across Atlantic, Bergen, Burlington, Cape May, Cumberland, Hudson, Hunterdon, Mercer, Ocean, Passaic, Salem, Somerset, Sussex, and Warren counties.

Participants Served:

Adults enrolled in DDD (21+), self pay may support individuals under age 21 as appropriate

Who Can Participate

Choosing the right Support Coordination agency matters. With more than 18 years of experience, Caregivers of New Jersey delivers person-centered planning that reflects real-life goals, needs, and preferences. 

Our Support Coordination services include:

  • Dedicated local support coordinators who know your county and community resources 
  • Individualized planning tailored to each participant’s needs
  • Assistance connecting with approved DDD service providers 
  • Ongoing monitoring to ensure services are working as intended 
  • Ongoing support to update and adapt service plans as needed
  • 24/7 on call support for urgent situations and emergencies

Our team works closely with individuals, families, and providers to ensure services remain responsive and aligned with evolving goals.

Who Is Eligible for Support Coordination

Our Support Coordination services are available to: 

  • Individuals enrolled in New Jersey’s Division of Developmental Disabilities program 
  • Residents of Atlantic, Bergen, Burlington, Cape May, Cumberland, Hudson, Hunterdon, Mercer, Ocean, Passaic, Salem, Somerset, Sussex, and Warren counties
  • If you are newly approved for DDD or looking to change your Support Coordination agency, our team can guide you through the process.

How to Get Started with Support Coordination

  • Complete the DDD Agency Selection Form and select Caregivers of NJ as your first choice Support Coordination Agency. If you need to make a change to your Support Coordination Agency, use this form.
  • Email completed form to [email protected].us or Mail the completed form to: New Jersey Division of Developmental Disabilities Central Office c/o SCA Selection Forms PO Box 726 Trenton, NJ 08625-0700 
  • After being assigned to Caregivers of NJ, aEnrollment Team member contacts you within 24 hours 
  • A dedicated Support Coordinator is assigned to you 
  • Individualized planning and service plan development is completed within 30 days 
  • Ongoing collaboration continues with the individual, family, and service providers
