Caregivers of NJ is one of New Jersey’s most experienced Support Coordination agencies. Using a person-centered approach, our Support Coordinators work closely with individuals and their families to develop individualized service plans, connect them to services, and support their goals for independence and quality of life.

Self-Pay DDD

Participants Served:

Adults enrolled in DDD (21+), self pay may support individuals under age 21 as appropriate

Union, Morris, Essex, Camden/Gloucester

What the Program Offers

  • Person-centered planning tailored to each individual’s needs and goals
  • Assistance identifying and accessing services
  • Guidance navigating the Division of Developmental Disabilities (DDD) system
  • Help coordinating multiple services and providers
  • Ongoing support to update and adapt service plans as needed
  • 24/7 customer support for emergencies

Who Can Participate

  • Individuals enrolled in New Jersey’s DDD program
  • Residents of Atlantic, Bergen, Burlington, Cape May, Cumberland, Hudson, Hunterdon, Mercer, Passaic, Salem, Somerset, Sussex, and Warren counties

  • Participants are assigned a dedicated Support Coordinator
  • Referrals accepted through DDD
  • Ongoing collaboration with the participant, family, and service providers
