FRN’s public health department aims to address the social determinants of health that impact individuals with intellectual and developmental disabilities, physical disabilities, and chronic conditions.

What Is a Transition Resource Fair?
A transition resource fair is an important opportunity for high school students with intellectual or developmental disabilities and their families as they prepare for life after graduation. These events bring together people, programs, and information that help clarify and make the shift into adult life more manageable.
At a transition fair, attendees can learn about the move from school-based services to the Division of Developmental Disabilities, understand what Support Coordination looks like, and meet local provider agencies that offer employment, community inclusion, recreation, and other essential supports. It is a chance to ask questions, build connections, and discover the resources that help students take their next steps with confidence.
Our next Transition Resource Fair will take place in Warren County on Saturday, March 28th, 2026, from 10 am to 1 pm at North Warren Regional High School. Families will have the opportunity to meet local providers, learn about DDD eligibility, and see how Caregivers of New Jersey can support them through the transition process.
