Support Coordination is a DDD funded service that helps individuals with developmental disabilities plan, access, and manage services that support independence, health, and community inclusion.
Yes. Individuals enrolled in the NJ DDD program have the right to choose their Support Coordination agency and may change agencies if their needs are not being met.
Support Coordinators will maintain regular contact at least once a month based on individual needs and must respond promptly to questions, service concerns, or changes in circumstances.
Caregivers of New Jersey offers 24/7 on call support to assist during urgent or emergency situations that impact services or safety.
Yes. Our team works with approved providers throughout Ocean County and nearby areas.
