Caregivers of New Jersey provides Support Coordination for individuals enrolled in New Jersey’s Division of Developmental Disabilities program. Understand Your Options

Review DDD services, approved supports, provider choices, and other available resources.

Support Coordination and DDD FAQs

What is Support Coordination in New Jersey?
Support Coordination is a DDD-funded service that helps individuals with developmental disabilities plan, access, and manage services that support independence, health, safety, and participation in the community.
Can I choose my Support Coordination agency?
Yes. Individuals enrolled in the NJ DDD program have the right to choose their Support Coordination agency and may change agencies if their needs are not being met.
What does the NJCAT have to do with my DDD budget?
The NJCAT evaluates support needs and is used to establish a DDD tier. That tier determines the annual budget available for authorized services through the applicable Medicaid waiver program.
What can a DDD budget pay for?
Depending on the person’s waiver program, assessed needs, Individualized Service Plan, and current DDD rules, services may include day habilitation, supported employment, behavioral supports, respite, transportation, assistive technology, therapies, individual or community-based supports, home or vehicle modifications, and other approved supports.
What is self-direction?
Self-direction can give an individual more choice over how certain eligible services are delivered, including who provides the support and how services are scheduled. The available model and requirements depend on the person’s program and approved plan.
How often should I hear from my Support Coordinator?
Support Coordinators maintain regular contact at least monthly based on individual needs and should respond to questions, service concerns, and changes in circumstances.
Can I switch Support Coordination agencies?
Yes. If your current Support Coordination services are not meeting your needs, you may select another approved agency. Caregivers of New Jersey can explain the agency-selection process and what happens after you are assigned.
What happens after I choose Caregivers of New Jersey?
After assignment, an Enrollment Team member contacts you, a dedicated Support Coordinator is assigned, and the team begins individualized planning and service-plan development. Ongoing collaboration continues with the individual, family, and service providers.
Which counties does Caregivers of New Jersey serve?
Caregivers of New Jersey serves Atlantic, Bergen, Burlington, Cape May, Cumberland, Hudson, Hunterdon, Mercer, Ocean, Passaic, Salem, Somerset, Sussex, and Warren counties.
